GIDDINESS SINCE 6MONTHS

HOPI:

PATIENT IS APPARENTLY ASYMPTOMATIC 11 YEARS AGO, THEN DEVELOPED HEADACHE IN THE OCCIPITAL REGION SINCE 11 YEARS WHICH GOT AGGREVATED SINCE 6 MONTHS DURATION - VARIABLE ASSOCIATED WITH NECK PAIN AND ON AND OFF LEFT EAR ACHE NOT ASSOCIATED WITH PHOTOPHOBIA ,NAUSEA , PHONOPHOBIA

GIDDINESS SINCE 6 MONTHS WHICH GOT AGGRAVATED SINCE 1 MONTH  - EPISODIC TYPE ,WHICH GETS AGGRAVATED ON SOLEPING AND RELIEVES ON SITTING

PAST ILLNESS: H/O SURGERY FOR VARICOSE VEINS ON RIGHT LEG.

H/O HYSTERECTOMY I/V/O TUMOUR

K/C/O HTN

NO PALLOR , ICTERUS, CYANOSIS, CLUBBING, LYMPHADENOPATHY , EDEMA

WITH K/C/O HTN WITH POST HYSTERCTOMY WITH RIGHT VARICOSE VEIN SURGERY

TREATMENT:

1. T. VERTIN 16mg PO/BD

2. T. ELIWEL 10mg PO/HS

3. T. TELMIKIND 20mg PO/OD

4. T. BPLEX FORTE PO/OD
